A world scale specialty
chemical manufacturing plant experienced a major explosion badly
damaging critical pressure vessels, process piping and equipment.
An investigation
into the causes and sources of ignition was performed. Corrective
designs were created and implemented. Environmental/safety regulations
were complied with.
ChemTech was retained
to investigate this incident, and similar incidents throughout
the world, and then determine the cause of the explosion and to
recommend solutions. ChemTech inspected the site, the equipment,
interviewed plant management, plant engineering and plant operating
personnel. Extensive photographs were taken, measurements of critical
items were taken, and demolition instructions were prepared. A
thorough, worldwide data search was undertaken for similar incidents.
A number of industrial companies were contacted for data relating
to process and mechanical designs.
The cause and type
of explosion was determined and verified with engineering calculations.
Extensive process design, control, and operational practices were
reviewed. Compliance with State environmental, safety, and labor
regulations were reviewed and met. Plant corrections were designed,
implemented, and re-start assistance was provided.
